This 65 years old male,with history of recurrent pruritus after few eruptions .Once these lesions become non - pruritic - they appear as these permanent lesions ..Diagnosis and treatment welcomed

Ddx: mastocytosis, papular urticaria with pih.t/t: antihistamines,LTRA's,+- steroids.

Tinea corporis ( Relapse or resistance ) Reassurance and counciling required because it takes more months . Levocet 5mg bd Itraconazole 200mg od Zocon 150mg weekly. Keto soap powder and lotion locally. Multivitamin and antioxidant orally. Keep the area clean dry and covered. For 3 months break for 15 days and repeat.

Resistant and relapse of Taenia corporis.d/d-scabies . Rx-1-epiclean-k soap for local bath daily. 2-Tab fexigra 120mg 1od. 3-zocon dusting poweder after bath. 4-Tab zocon DT 150mg 1od 5-Tab b complex 1od �8 weeks.

Appears mixed infection,of scabies & fungal

These eruptions are crusted scabies Rx tab azithromycin 500mg 1od for 5days Tab Levocetrizine5mg 1bd Tab ivermectin 12mg 1stat and one more after 15days Permithrine oint 5%30gm to be applied all over body at a time and to be repeated after one week

Extensive tinea incognito Can be confirmed by koh examination Patient history is important If tinea is confirm then stop steroid in any form Treat tinea as per protocol
