Pt age 27 yrs/f C/O dysmenorrhoea USG report shown uterine fibroid Pt is unmarried Please suggest management and appropriate treatment of uterine fibroid??

Only symptomatic treatment required. After marriage laproscopy and myomectomy may be required for future pregnancy for better outcomes. Now texakind mf / spasmodart may help for pain reduction and reduction in blood loss. GnRhagonist or selective progesteron or estrogen receptor modulators may help.

Find out other cause of dysmenorrhoea as a posterior wall fibroid is not to be considered for. Better to go for symptomatic treatment

Myomectomy should be preferred if relatives agree as she is unmarried

This is the point that she is unmarried that's why her family wants to take her on medication only

27 yrs unmarried women with dysmenorrhea showing small uterine fibroid, a possibility of associated minimal endometriosis needs to be kept in mind. According, my advise would be for Duagnostic laparoscopy and if desired for removal of myoma in the same sitting. Strong advise for early marriage..

U may suggest letrozole/mifepristone(low dose) /gnrh agonist for size and symptoms reduction bt explain ur patient first tht after stopping medical managmnt size again ll be same/increased.Acco. to me Lap ll be best as it also improves her future pregnancy chances.
