14 yr boy having sever iciting all over the body with burning.what is diagnosis

60 years old male patient normotensive nondiabetic with progressive lesion on both palms and soles. It started as a solitary papule on the Palmar aspect of left wrist. He is a farmer and uses chemical fertilizers as a routine part of his job. Please elaborate the dx and management. Thank you.
