Its a classic feature of Globulomaxillay cyst (inverted pear shaped radiolucency).. D/d - KOT,lateral periodontal cyst, radicular cyst My suggestion- even if u find the cyst is small.. decompression would be good since it can help in preserving the pulp vitality and pdl integrity of the adjacent tooth.it requires opening of a small surgical window and is best conservative method. Lesion slowly decreases in volume with gradual deposition of bone without much adverse effects to lateral and canine... Best wishes doctor.

Globulomaxillary cyst Treatment plan - surgical enucleation of the cyst and curettage Check the vitality of 22,23. Any intraoral pictures?

Globulomaxillary CYST Enucleation will be best if done
