Raised IgE levels with Deranged liver Enzymes.

Raised Liver Enzymes with Increased IgE levels. What's the possible diagnosis? Chief Complaints Lethargy History 43 year old male, k/c/o DM2 for 10 years on OHG's and HCM. Chain smoker, quit smoking a month back. Investigations Sgot - 251 Sgpt- 102 IgE - 228 ( no signs of Atopy, no eosinopholia on USG Abd - Mild Hepatomegaly with Fatty infiltration. Triple serology Negative. HB - 16 LDL - marginally raised TLC - 7,900 U/E, KFT, CBC and all other parameters normal

This is a case of Type 2 DM with mild hepatomegaly with fatty liver ?? NAFLD Rule out Cirrhosis and HCC. Fibroscan/ CT scan of abdomen is required to rule out HCC . Rule out CAD by doing Fasting lipid profile and ECG and 2D ECHO.

Raised liver enzymes in a known diabetic and smoker with negative viral serology, first differential would be NASH. USG finding of diffuse fatty infiltration also supports that. Advice for weight reduction of atleast 5% through regular exercise and diet and control of glycemic status. If still enzyme level remains elevated after 3 months, then refer the patient to a gastroenterologist for liver biopsy to evaluate further for other causes like AIH.

give mrudu virechna with trivrit and after that start tab Nirocil 2tds along with arogyavardhini vati and puarnavadi mandoor vati...and give fat free diet ...no spices at all.

IGE level found elevated in about 55% with acute and chronic liver disease of different etiology .PT is Diabetic for 10yrs,chain smoker , elevated SGOT,SGPT level ,LDL increased ,USG finding in favour of NAFLD/NASH Control of Diabetes,Life style modification,caesation of smoking Hepatoprotective drugs Biopsy liver ,for confirmation
