TREATMENT : PREVENTIVE :- 1. Advice to wash the private parts of both the parts immediately after the coitus. Also advice passing urine within 10-15 minutes to flash out bacteria which entered into the urethra. 2. Ask to take bath in shower instead of bucket water. 3. Advice to clean the perineal area with clean piece cloth from front to back after each urination and also after bathing. Never to be wiped from back to front. Also advice to wash the genital area after each urination. 4. Scented product like scented soap, spray, powder etc to be avoided in the genital area as its damage the protective skin flora which prevents ascending bacterial infection from perinium into the bladder. 5. Advice intake of adequate water as it flash out bladder infection if any. 6. Ask not to hold urine for long time and will pass when feels the urge. Bladder to be evacuated fully. Retained urine grows bacteria faster. SUPPLEMENT :- 1. Cranberry extract, D-Mannose are the supplements which are found in capsule form (Cranfit, Tractfree etc brands contain Cranberry 300 and D-Mannose 600) prevent attachment of E. Coli with bladder wall which come from anal area to enter the bladder through the urethra. Loosen bacteria thus easily flashed out during urination. 2. Probiotics (Brands - Darolac, Sporolac, Lactobacil etc) restore genital flora and maintain acidic pH of it which acts as a guard to prevent ascending infection from anal area to bladder via urethra. SYMPTOMS RELIEF : 1. Urine alkaliser (Disodium hydrogen) :- Keeps the urine alkaline to reduce burning urination and dysuria. However it doesn't treat the infection. Not used along with Nitrofurantoin as it works in acidic urine. 2. Flavoxate :- Antispasmodic for bladder muscle and sphincter this reduce symptoms like dysuria, burning urination, Frequency etc. ANTIBIOTICS : Recurrent UTI is considered as Complicated UTI. So antibiotic should be chosen on the basis of Culture Sensitivity report. If C/S is not available then treat with..... 1. Mild to moderate cases :- Nitrofurantoin, Fosfomycin, Cefixime, Fluroquinolones, Co trimoxazole etc. 2. Moderate to severe cases :- Inj Amikacin or Gentamicin, Inj Ceftriaxone, Levofloxacin etc.
