Red patches

54 year old female. Unusual rash began with 2 spots on the shoulder (as shown in picture titled early image) about 1 week ago. She has developed new lesions daily, primarily on the trunk and inner thighs and now the arms as well. The lesions are annular and raised and well demarcated but do not have central clearing or any scale. She has no systemic symptoms and no recent new exposures (such as meds, soaps...) Patient complains of moderate itch and slight burning sensation.

A 64-year-old man presented with a two-year history of this asymptomatic skin condition, which was symmetrically distributed over his trunk, arms and legs. Laboratory tests demonstrated a raised fasting blood glucose level. What is the most likely diagnosis?
