27years migrant worker working as housekeeping women, presented to clinic with left loin pain, got USg abdomen shown Renal stones. Having pain controlled by ketrol. Not affordable for surgery or admission as being single mother has 3 children at home. Can someone plz guide
patient was given Inj. ketrol and she got settled in two days, advised her rest and supportive care. Started her on Tab.Cystone 2 tablets two times a day. she is doing quite well.

Present problem is due to ureteric calculus in distal part probably at uv junction hence hydroureteronephrosis This can treated with hydrotherapy Iv fluids Inj diclofenac Inj Ceftriaxozone Inj dexamethasone Inj lasix Rest supportive Once pt settled down or calculus passed or excreted in bladder shift on oral treatment Tab calcury 1tds Tab gokhsharadi goggul 2tds Varuna-d-kwath 2tsf Anti spasmodic tab meftalspas Oral antibiotics like tab cefixime+ofloxacin if needed Cap nefrosave 1tds Keep followup
Zifi.CV...1 tab BD.x 7 days.. Cystone tab 2 tab BD. Potassium Citrate ..2 TSF once diluted with water. Increase the water intake. For pain MAFTAL SPAS.1 tab BD....SOS.

it is difficult for a stone to get passed of with ashmarihar medicines. moreover hydronephrosis development mens the the calculi could be detrimental to the kidney health. you may continue with ashmari har medicines like kulatha, gokshura, hajral yahood bhasma but please also keep a check on renal function test and get Intavenous pyelography done. if the patient does not want surgery then lithotripsy is a good option for the patient.

6 year old female presented with complaints of mild left sided flank pain No dysuria, no increased urinary frequency, no hematuria etc. Medical expulsive therapy with Tamsulosin cant be given as it is a child Dietary advises can be given along with plenty of fluids especially water For alkalisation, I have prescribed a syrup of potassium citrate, as well an analgesic on sos basis. Do you still think she should undergo urine cs, r,m, S Urea, S Cr, NCCT KUB? Your line of management for such patient to tackle hydronephrosis along with flank pain?
