As the pt is having pain abdomen rt flank radiating to anteriorly and posteriorly suggestive of renal colic. Rise of urea 62 may be due to less intake of water leading to dehydration . D/D Acute appendicitis Blood count differential count to be seen As due to lockdown USG could not be done treat conservatively IV drip to continue Inj monocef 1 gm I'm bd Inj antispasmodic an sedation may be given for pai releif . If the symtomps subsides wait for USG to differentiate from appendix If not subsides admit in hospital as an emergency.
