Right leg inflamed red and bulbous growth with multiple nodules

Man 47 yo with pain at RLAQ at palpation looked like appendicitis and skin of right leg inflamed red and bulbous growth with multiple nodules. Pics attached...Suggest the treatment?

Being surgeon you are suggesting look like appendicitis while post operative scar is seen if appendicitis is suspected than what for this scar is Do you suggest surgery was for some other indication like renal calcus if that is so go for usg abdomen to look underlying cause Yes on face of it this is sepsis So far leg eruptions are concerned Looks to be periportitis D/d pyoderma

Post operative sepsis. BS antibiotics orally. Analgesic antiinflammatory orally. Multivitamin and antioxidant orally. Clean area with diluted betadine lotion apply betadine lotion dressing . If not responding to oral than iv and debridement under LA and ASD with betadine lotion. Needs further investigation and evaluation to conclude.

SUGGESTIVE OF SEPSIS A D PYODERMA

Sepsis.. Pyoderma..

But did he get operated? What was the cause for surgery? If there is non-healing of surgery site with leak and the lesion in legs looks like evolving pyoderma gangrenosum, then this may be Crohn's disease. Has any imaging been done? Get a dermatologist reference for the lesion.
