Right-sided weakness

A 50-year-old male presents with the complaint of right-sided weakness and slurred speech. CT images are attached.

NCCT brain is suggestive of infarction in the territory of left middle cerebral artery with subsequent weakness of the opposite i.e right side . It seems - the window period of 4 hours is over and thrombolysis may not be effective. Give tab Atorvastatin 40 mg + Clopidogrel 150 mg + Aspirin 150 mg O.D along with supportive treatment Addition of tab citicholine + piracetam B.D for 2- 3 weeks will help

Early hypodensity left temporal with Dense MCA sign left side suggestive of hyperacute infarction left MCA with thrombus left MCA. This pt needs thrombolysis if less than 4.5 hrs. .In view big thrombus, the pt needs mechanical thrombectomy if less than 6 hrs. If the window period is over and then go for routein management.

Left temporal infarction. CT brain angiography. Needs further investigation and evaluation to conclude and line of treatment.
