Ruptured Liver Abscess

A case of ruptured liver abscess with intestinal ascariasis. Chief Complaints Severe pain abdomen Nausea, vomiting, anorexia, constipation High grade fever with chills and rigors History Chronic alcoholic Vitals PR: 134/min BP: 90/60mmHg Temp. 102.4°F RR: 34/min Rest WNL Physical Examination Per Abdomen: Tense, Mildy distended, Generalized tenderness Guarding and rigidity present Bowel sounds absent Investigations X Ray abdomen: multiple air fluid levels, with ground glass opacity. USG: ruptured liver abscess with intestinal worms 🪱 CBC (anaemia with leucocytosis) LFT and RFT: essential normal ( mildly deranged) Diagnosis Ruptured Liver abscess with ascariasis Management Exploratory laparotomy with drainage of liver abscess with thorough peritoneal lavage, with enterotomy and milking and extraction of intestinal worms (ascaris) and drain placement.
