Concluded answer

Cxray shows absent bronchovascular marking on Rt apical lung field and left hemithorax. Fibrotic lesions seen in rt mid zone and left paracardiac region. Slight mediastinal pull towards left. Possibly destroyed lung with large emphysematus bullae or pneumatoceles giving this appearance. Old Infective sequelae is already mentioned in the history. It would be great to see the HRCT thorax images.
