Sepsis with leucocytosis with AKI with Jaundice

Breathing difficulty, generalized weakness, fatigue,fever, Chief Complaints Gabhrahat, breathing difficulty,fatigue Fever on/off History Patient is high tlc count , antibiotics given as per C/s report(E.coli in pus c/s) ,but not much effect Vitals BP 125/85 Pulse 92 Spo2 96% RR 26 Investigations Reports attached Blood c/s sterile Pus c/s -E.coli TLC on 11/4-54000, in evening-49000 12/4-61000, evening-45000 Today 30000 B2M-3950 Diagnosis Sepsis with leucocytosis with AKI with ALD Management Currently patient on Ini Targocid Inj Mero Patient clinical stable Please give ur opinion on this case

Pt is neither hypertensive nor diabetic But having AKI as a result of SEPTICEMIA (resistantE coli) Focus of sepsis indicated by NCCT as gluteaus major along s1s2s3 vertebral bodies Poly serocitis bcz of septicimia as pleural effusion and ascitis is noted Deranged LFTS hepatosplenomegaly KFTS shows high bul and sr creatinine This is likely oligouria or anurea Yes anaemia is by virtue of sepsis Overall it is septicemia with AKI Rx inj meropanum 1gm 3to4 times Inj tazobactum Inj lenazolinid Inj lasix Inj dexamethasone Sos renal dialysis

Aki on ckd with acute hepatitis and anemia dur to renal disease with decreased rbc and hb also please mention duration of each complaint so that it can be a sure diagnosis .if breathing difficulty is acute onset pe can be suspected also getvan abg for lactates as pt is having neutrophilic leucocytosis with tachypnea high chances of dvt and pe
