Sever iron deficiency anemia

31 year old female, bmi > 40 C/o chronic fatigue hence inability to exercise daily and ,palpitations. Blood work shows low serum ferritin alow hb levels along with others. Iron supplements have not shown any results for a long time. Please provide ur opinion and suggest some iron supplements that work. Reports attached

Mild iron deficiency anemia ,with obesity . Need to rule out parasitic infestations, blood loss in GI tract , menorrhagia. Usually Fesovit , 1 cap twice daily is effective when given for 3 months. Monthly CBC monitoring needed. If even after 3 months of oral iron therapy , iron stores are not restored, parentral IV iron therapy will be appropriate. . Effective measures for weight reduction are important. Cardiac evaluation is needed.

First of all she is obese and overweight having bmi>40 hence problems are there. She needs weight management Low hb with low MCH suggest megaloblastic anaemia. Check vit b12 if deficient suppliment same She has to observe diet restrictions Active exercises Change in lifestyle Blend balanced diet

Iron deficiency anaemia Find out the cause .. Give I/V Iron .. Do KFT and USG whole abdomen to rule out CKD . Do VIt b12 levels to rule out combined deficiency. If not improved Do UGIE to find out any bleeding. Rule out coeliac disease

Check for stomach pathology, I think iron absorption is distrubed, do stool routine, check for occult blood
