Severe continuous headache
Chief Complaint I have a relative 53 y/o female, who has been complaining of severe continuous headache for 1 week. She explained headache as 7/10. It was not relieved completely with painkiller. History She had similar episode 2 months back also but resolved with a painkiller. No family history of such headache. She is non smoker and non alcoholic. Examination She is a healthy female. Neurological examination-normal. Investigations Brain MRI shows a large lesion in the left occipital region suspicious of tumor. Chest and abdominal radiograph show no pathology. Treatment What should be the course of treatment?
? HEADACHE..WITH.. BRAIN PATHOLOGY.. NEED'S.. NEUROLOGISTS OPINION.. MEANWHILE TREAT SYMPTOMATICALLY..
Brain pathology ? Needs neurosurgeon opinion and till conclusion symptomatic treatment. Inj tramadol im can be considered for releasing pain if uncontrolled pain.
How big is the lesion? Can you put the images Need to biopsy the lesion first. It could be a glioma or CNS lymphoma, which can be kind of figured out with imaginig, although a biopsy is recommended before starting therapy. If this is a glioma then treatment is usually resection if possible (with temporal lobe lesion that seems likely) followed by radiation + chemotherapy. If this is CNS lymphoma, then treatment is chemotherapy with high dose methotrexate based regimen.
