A female aged 34 years who is 32 weeks pregnant Chief Complaints Refractory, persistent Vomiting (x 15 days) Small volume, non bilious Pain left side of chest Physical Examination Nothing unusual Investigations Xray chest demonstrated some hollow viscus pushing the left hemidiaphragm

Diaphragmatic herniation lt side likely due to advanced pregnancy There is density seen upper part of mediastinum overlapping carina likely rudimentary thymus or lymhoma need to explore

Rt paratracheal opacity seen. Cardiac shift towards right side. Left sided linear opacity and leucencies noted below it. Needs evaluation for left diaphragmatic paralysis or gastric distension.
