65 years old male,presented with lower urinary tract symptoms since 2 months(a/w increase in frequency,urge incontinence,nocturia, poor and intermittent stream of urine and urinary retention), burning micturition since 10 days, and fever since 3 days. No h/o weight loss, evening rise of temp,chronic pelvic pain,lower abdominal pain,hematuria/pyuria. K/c/o Diabetes and HTN since 5 years. USG findings(image attached):- enlarged prostate and left kidney atrophic. No hydronephrosis/calculi. Urine culture:- positive for E.coli. The patient is diagnosed with BPH with UTI with HTN and DM2. Please discuss the management.
