spot diagnosis and treatment plz 50 years /male ....sever itching since 10 dyas h/o allergy 10 years
Allergic contact dermatitis with deep tissue infection ( cellulitis).. LSC on ankle. Ad- broad spectrum antibiotics. Topical betamethasone. Oral vit E. Oral Hydroxyziene 25mg.
2 types of lesions seen LSC Ankle region and papular lesions surrounded by erythema on upper lesions, could be allergic dermatitis ppted by insect bites
Pt is kco allergy for 10yrs but allergen is not clearified thst means unknown allergen given picture is of L S Con ankle and rest of leasion are pappular secondry to allergen and shows sec infection Rx lsc lobate-s oint twice daily Tab Levocetrizine5mg1bd for 5to7days Tab dispred4mg 1tds and gradually tapper off for pappular eruptions fucibact oint twice daily . Review after one week Rx for lsc to be continued and to be reviewed after 1month

Pt aged 44 yrs....C/o generalised itching....Since 8 yrs....Daily itching....Taken treatment at many drs no Response and well Known dermatologist treated no response.... Pt is non diabetic/No thyroid problem/No Anaemia/No Scabies/No Liver problem/No renal problem/No Tension or Hysterical/No HIV/No Hbsag positive/No Drug Related Itching/Non HTN/No Food Related Allergy.....Dermatologist given her steroids/Dapsone tabs etc her weight increased but no Symptoms relieved....So pt is Very much Fed up of this along with family members.....No Relief of Itching.....So Challenging One pls share ur Opinions fast......If u feel diagnosis then tell Related Investigation,Treatment and prognosis...Must.
