Sinus tachycardia with hr-140/min, with lv strain with left ventricular hypertrophy with p pulmonale. with St depression in lateral leads sec to lv strain and ischaemia. congestive cardiac failure with accelerated hypertension. propt up oxygen nebulisation nitroglycerin intravenous infusion diuretics degoxin intravenous cardiac monitoring sos noninvasive ventilation.

Ventilation is a important condition depend upon condition nothing vasive or invasive

Pulmonary oedema mostly due to lvfailure Tachycardia with inferolateral infraction sir my opinion is send TropI and BNP to rule out MI AND DIASTOLIC HEART FAILURE AND DO CHEST XRAY 2DECHO

Rx: inj NTG to maintain BP not less than140/90 Inj lasix to maintain volume overload O2inhalation5-6lts /min as per abg status For ischemia- tab ecosprin300mg stat,tab clopidogrel 150mg stat,tab atrovastatin 40mg stat..post 6hrs if required add tab cordrone 150mg/tab digoxin.25mg ever altrnte day a week..b blockers CI..tab lasilactone/Tide plus 50/20. 1-0-1 prophylactically..

It's a case of supra ventricular tachycardia.

LAD, Sinus tachycardia, Rt Atrial enlargement, LVH with strain.

My opinion sir give lasix and cpap and NTG AND ANTIPLATELETS AND IVABRADIN B BLOCKERS AT THIS SITUATION AVOID SIR
