Graves Disease

Suspected case of Graves disease r Chief Complaints A 36 yr old male attended Mopd with Palpitation and anxiety with diaphoresis since 1 month. O/E Pt was having Postural Tremors on outstretching his hand,Proptosis and increased PR of 116/min. No Neck swelling,No Dry eyes,No Diplopia or visual problem. Von graffe, mobius sign,stellwag sign and other signs are positive. Routine Ix was advised . Reports arrived showing Low TSH less than 0.05. ECG showing Sinus Tachycardia. Graves ophthalmopathy, also known as thyroid eye disease (TED), is an autoimmune inflammatory disorder of the orbit and periorbital tissues, characterized by upper eyelid retraction, lid lag, swelling, redness (erythema), conjunctivitis, and bulging eyes (exophthalmos).

Proptosis outside protruding eyeballs Tsh low, hyperthyroidism FT3 FT4, anti TPO antibodies needed

TFT TSH receptor antibodies. Thyroid Nuclear scan Early antithyroid therapy. Consider betablokers Folic acid along it.
