Testicular mass
Chief Complaint A 42 y/o male presented with right testicular mass for 4 months which has started to increase in size for the past 20 days. He also complaint of pain over the area. Examination Physical examination shows a large testicular mass which is tender on palpation. Investigations Gonadotrophin, fetoprotein, and human placental lactogen were wnl. USG shows a 1.5 cm * 1.2 cm mass. A left radical orchidectomy was performed, histopathological report is pending. Treatment Should we go for retroperitoneal lymph node dissection also?
Seminoma Wait for histopathological report. Needs further investigation and evaluation and then after assessment go for dissection of lymph node advisable. Local support and NSAID for pain.
First diagnosis is Seminoma. Confirm any involvement of Malignancy by HPE. Proceed for surgical intervention accordingly.

Is this seminoma or Non-seminoma? What did CT of the abdomen/pelvis and chest show? RPLND is a very complex surgery and should not be done without good reason. Need for RPLND depends on the path report, size of retroperitoneal LN. Usually in seminoma, you do no need to do RPLND as it is highly chemo and radiation sensitive, and enlarged LN can be treated with these modalities depending on the size of the LN.

This is an x-ray chest PA view from a 26-year-old boy who presented with a left scrotal swelling for 6 months and low grade fever for 2 months. On examination, built and nourishment is below average, afebrile. No lymphadenopathy, clubbing. Mild pallor present. A swelling of size 2 X 3 cm is found in left scrotum arising from left spermatic cord, non tender with no local rise of temperature. Rest of the systems WNL. The case is open for discussion.
