Diabetic Foot.
Uncontrolled T2DM (Drug Defaulter) with Diabetic foot and D. Nephropathy? Chief Complaints A 55 yr old lady has come with wound over Rt foot. H/O maggot removal from the wound 1 week back. Also history of fever 2 days back. She is a k/c/O T2DM on irregular medication. No Associated features or H/O HTN, Thyroid disorder, substance abuse etc. Pt was admitted and Routine Ix along with Covid RAT was advised. Wound was debrided and sent for C/S. Her Ix showed sepsis , low Hb, proteinuria and pseudomonas sensitive to Amikacin in debrided tissue culture. Fundoscopy was done showing no Retinal changes. Rest Ix are normal. Pt is presently doing well on medications.

Diabetic foot Uncontrolled and irregular treatment with noncompliance Your approach is appreciable Since organism is identified and sensitive to amikamicin so carry on as pt is responding Keep control of diabetes and possibly on insulin
Diabetic foot , ? underlying PPKD, needs debridement and dressing, rest for the limb, antibiotics. Uncontrolled DM , needs insulin for tight sugar control.. Anesthetic limb, needs foot care, MCR footwear, neurotrophic vitamins. Suggest arterial color doppler of the limb, and correction of vascular occlusion and stenting Correction of anemia., hypoproteinemia. Xray of foot to rule out OM .. Needs cooperation of patient and attenders, since this is a time and resource consuming exercise. Patient education is important. .

80 year old male with history of DM, PVD, related to non healing diabetic foot ulcer. Now has this area on top of left foot. Has had this scab since earlier this year. Has not grown in size. No odor, no drainage, no pain. Reports no fevers. Has mild neuropathy to LLE. Suggestions on any further treatment options/tests? Worried about what may be presenting underneath the scabbed area with his Hx of PVD/DM.

A 45 year old female presented with this non healing wound due to toe-ring over second toe anterior aspect of left foot for past 10days. She has been getting injection cefotaxim twice daily from another practitioner. She is a known case of T2DM. Weight - 55 kg Height - 165cm BMI - 20.22 Her present RBS is 260mg/DL I have asked her to get HBA1C, FBS AND PPBS, which I will update soon. Meanwhile sir what antibiotics, OHA and topical application would be best suitable for such patient. @Sepuri Krishna Mohan Sir, I need your valuable advise too.

A 63 year old female presented with non-healing ulcers over bilateral soles of 1 year duration and surrounding skin thickening of 6 months duration. She is a known case of diabetes mellitus on oral hypoglycemic agents for the past 10 years. She gave history of repeated plantar ulcers and reduced sensation over bilateral feet up to the ankle region. Comment on the diagnosis and mx.
