Untreated Gastric adenocarcinoma

Chief Complaint A 67y/o male came for cough, worsening dyspnea, and anorexia for 1 month. History He was diagnosed with gastric adenocarcinoma 4 months back, No surgery was done till date. Vitals BP:110/70 mmhg, Resp rate: 17 bpm, Pulse: 69 bpm, Temp: 98.2 degree F. Examination Respiratory sounds were reduced in both lower lungs. He has b/l leg weakness and sometimes leg paralysis also. Oxygen saturation was 85% on room air and 96% with 2 l/min oxygen administration. He also demonstrated Investigations Chest X-ray shows enlarged cardiac shadow. ECG showed pericardial effusions Lab reports shows raised serum liver enzymes. Treatment What is your opinion on this case?

Untreated c/o gastric adenocarcinoma Now facing the jingle bells of complications of cancer Pericardial effusion and decreased AE in lung fields Difficult to treat Needs Diuretics Sp02 saturation with ventilation support Deranged liver enzymes likely metastasis Now this case is to receive chemoradiotherapy if helpful
