Subacute appendicitis... can go with conservative approach with antibiotic and supportive treatment... Lap appendicectomy is ideal approach here.

The lady is having recurrent pain in rt lower abdomen and rt inguinal region and both time USG report indicating a case of recurrent sub acute appendicitis and no othe pathology has been mention .Pain in rt inguinal region is not a typical site of pain in appendicitis but positions variation of appendix may be pelvic type giving pain in lower down inguinal region. Untill unless appendix being removed it will not be possible to rule out appendicitis. So appendicectomy should done after all relevant investigation if necssary followed by HPE.

2yrs back the diagnosis was Subacute Appendicitis. So this time repeat USG of Abd and Pelvis should be done along with other routine investigations like CBC, ESR, FBS, Urea, Creatinine, Urinalysis, Na, K, and Straight X-ray Abdomen. If you want get a better outcome for this patient of recurrent appendicitis, then you should go for Appendectomy.
