Concluded Case

What can be the diagnosis 58y/f prsents with below mentioned complaint

K/c/o HTN,with sudden epigastric pain,relieved on ivf and pentazocin,but mild occasional pain persist. Usg wnl.cbc amylase normal lipase slightly high. Na and k normal. Also dizziness and burning feet present now. Can anyone think of any diagnosis?

I agree.endoscopy should be performed.

As you mentioned USG is WNL so epigastric pain may be due to ulcer and it can be treat symptomatic. But burning sensation of foot is should be rule out , it may be due to 1) Diabetic neuropathy. 2) Hypothyroidism. 3) Kidney disorders. 3) Deficiency Of Vit. B-complix 4)chronic tobacco or alcohol consumption.

Most probably epigastric ulcer of stomach. Advice some PPI Antacid Avoid spicy food Drink cold milk at night If not subside Go for endoscopy
