What is best technique to extract horizontal deeply impacted lower 3rd molar please
Deeply impacted implies Pell and Gregory class 3C. 1)Nerve block LA(IANB+Lingual+Long Buffalo/ Mandibular block) 2)Full thickness flap extending from 1st molar to retromolar region, take care to avoid damaging the lingual nerve. 3)Bone guttering with thorough irrigation 4)Coronectomy 5)Make a purchase point on the distal root which lies superior to medial root in horizontal impactions, lunate using Cryer elevator, repeat for medial root. 6)Irrigate thoroughly, grind away any sharp spicules of bone. 6)Suture using interrupted suture pattern with non resorbable suture. 7)Injection diclofenac sodium and dexamethasone to control pain and post op swelling. Give post op instructions and medications. Recall after 1 week and review.
