What is the diagnosis?

This patient is 12 year male child..with slowly developing spastic quadriparesis, with massive Anasarca..slightly decreased due to diuretics, with Protrusion of tongue since March.All limbs are spastic and externally rotated limbs,not able to swallow only liquid is being taken.CT brain Normal.all other Investigations like Echocardiography and and Normal blood profile Normal.what are probable Differential diagnosis??

Has had normal development until this illness.normal vaginal term del.no adverse birth events
