what is the diagnosis?
A 70 year old woman presents to her family doctor complaining of chronic cough with occasional chest pain. She states that when she exerts herself or coughs, she experiences pain in the right side of the chest with difficulty breathing. She states that she lost 20 pounds over the last 3 months. She has been a single landlord earning passive income since she divorced 30 years ago. She has never smoked and drinks alcohol socially. She denies any marijuana, cocaine, or heroin use. She has not traveled anywhere in years, preferring the comfort of her home. On exam, the patient is an emaciated woman with an unremarkable cardiac exam. Auscultation of the lungs reveals decreased breath sounds and dullness to percussion over the right upper lobe. Chest radiograph is shown in the picture. What is the most likely diagnosis?
Yes it is tumor rt apical zone with h/o wt loss malignant ?adenocarcinoma lung To differentiate needs biopsy

61yr / F, Diabetic on treatment, chronic weakness for last 18months, presented on 23/8/18 with SOB on effort for 2 weeks, Loss of Appetite for more than a month, she is occasionally having feverish. She had left sided massive pleural effusion in 2017 treated at other centre. She is having some skin lesion recently with SOB. Her CXR of 2017 and Pleural Fluid report, PAP report, new CXR, Echocardiogram, USG, FBS145, PPBS235, Creatinine1.6. One Litre of Haemorrhagic pleural fluid has been aspirated today and sent for Analysis. She is having mild Anaemia( as per records) since last 18 months. Please discuss the possibilities and Approach.
