What is the most probable diagnosis and management

there case of female patient of weak built aged about 21 years. has bilateral mild flank pain from last one year. the pain is of migratory and fluctuating nature and it is associated with 1. pain during micturition 2. difficulty in hearing 3. loss of appetite 4. Fatigue 5.headache Chief Complaints Flank and lower back pain Investigations Ultrasound repot is normal

She is a case of complicated urinary tract infection: Pyelonephritis, as she is having flank pain accompanied with dysuria with CBC revealing mild Leucocytosis and Urine R/M revealing pyuria. She would also need a USG - KUB to see if any stone or hydronephrosis etc. as well as Urine CS too to check if bacteruria is present along with drug susceptibility. She can be started on following drugs - Inj Ceftriaxone 1gm IV 12 hourly plus If S. Urea and Cr is fine, can start her on Inj Amikacin 500mg in 100ml NS 24 hourly Alkasliser syrup Inj Pantop 40mg 24 hourly

UTI?? ENTERIC FEVER?? Do usg -abdomen pelvis if any stone bcz complaining of flank pain.. Also possibility of chronic appendicitis. START WITH TAB.ZIFI-CV 200 BD FOR 5 DAYS TAB.FLAVOXATE BD FOR 5 DAYS SYP.CITRALKA If ureteric stone less then 7 mm Start urimax 0.4mg h/s With varunadi kashay 2tsp TID.
