What's the problem?
9 year's old girl came with parents inflammation and swelling of left hand elbow joint. Chief Complaints Pain in left elbow joint. History History of traumatic from bicycle Physical Examination Inflammation and swelling of elbow joint. And severe pain feeling when the joint flod or move hand. Investigations X-RAY attached Diagnosis Please suggest Management Please suggest
Bony architecture look normal How old is the injury ? If it is a treated case, possibility of myositis Arm sling Active finger movements Gentle active exercises of elbow within painless range Indomethacin is especially helpful in these cases Reassess response to treatment after one week Other possibilities are 1 Traumatic synovitis 2 CRPS 3 Juvenile rheumatoid arthritis 4 post traumatic inflammation of soft tissues Usg evaluation will help CBC esr RA factor Uric acid Crp levels also may help in finalising the diagnosis
X-RAY STUDY.. NO BONE LESIONS.. NEED'S.. SYMPTOMATIC T/T.. FURTHER EVALUATION..
X ray normal Ligament injury Rx 1. Oral anti infammatory x 5 - 7 days as necessary. 2. Immobilization of part x 3 weeks.
What are these tiny pin point white dots? (X ray has rashes????) Rest is wnl
Possible synovitis due to trauma
POSSIBLY . POST TRAUMATIC ...INFLAMMATION
No obvious bony injury ,soft tissue injury Fat pad sign seen. Suggest MRI ,ortho opinion

11 years old female, pain in joints from 1 month moderate, inflammation of elbow,wrist, knee, ankle from 20 days, fever high grade from 4 days, deformity of metacarpal joints from 3 years, TB contact +ve (mother & sibling). O/E: inflammation of elbow, wrist, MCP, wrist, knee & ankle joints symmetrical, warm & tender, nodules on MCP, joints pigeon chest, chest b/l clear, GCS 15/15, anterior fontanelle wide open. Ix: WBC 12, HB 10, plt 394, Spinal XRay shows mild Kyphosis, Platyspondyly at lumbosacral joint, ASOT -ve, XRay both hand & feet shows soft tissue calcification, bone density mildly reduced, USG joints: no joint effusion. Diagnosis & Management?

35 years old female suffering from recurrent painless nonitchy swelling for last 5 years. No h/o respiratory distress. No family history of such lesions. She is a known Hypertensive and for that she is on Telmisertan 40 mg/day. Patient has been treated with Levocetirizine 10mg twice a day for six months and Monteleukast 10 mg at night for six months, but no relief. Only responding to short courses of systematic Steroid 2 mg /kg with tapering dose. 1). Diagnosis 2). D/D 3). Investigations 4). Management. Please give your answers in chronological order.
