Diabetes affects the innate and adaptive immune responses necessary to control TB infection. There is reduced ability to clear infection could conceivably lead to TB-related poor outcomes among people with diabetes. Therefore it is of great importance to achieve strict control of blood sugar in patient with diabetes Use of Metformin and insulin would be encouraged in control of blood sugar Metformin and antituberculosis drugs do not have significant drug interaction, therefore it can be safely used.

Rifampicin has drug interactions with almost all antidiabetic agents. Consider glipizide as first choice sulfonylurea to minimize interactions. Metformin, alfa glucosidase inhibitors, and GLP1 receptor agonist like exenatide has no contraindications of given with Rifampicin. Isoniazid has interaction with insulin.

Diabetes and tuberculosis should be treated aggressively with insulin. In case a person with diabetes and active tuberculosis is poorly controlled on oral hypoglycemic agents, it is necessary to switch to insulin. The choice of insulin regime should be based on efficacy, safety, tolerability and convenience.
