Start with 1)Methotrexate 5 mg once a wk with Folinic acid supplements 2)Deflazacort 6 mg tds/bd , taper after 4 wks not more than 12 wks 3)PPI 4) Calcium Vit D tabs 5) Aclofenac paracetamol for few weeks until pain resides then gradually taper and stop over 6 months-1 yr 6) Low HB is common , start Ferrous sulphate VIMP : Monitor SGPT + Creatinine + ESR monthly for a year. When ESR < 50 you can gradually decrease the dose of steroids. Dr.ortho oil for external use

6 years male child presented with cough and fever since 10 days. H/O abdomen distension since 5-6 months which gradually increased over 2 weeks. H/o joint and limb pain since 5 days, h/o loss of weight and loss of appetite + O/E thin built, toxic look, conscious, oriented with stable vitals accept mild tachypnoea. Tongue not coated , throat not congested.angular Cheilosis with some impetigo + Pallor +++ No lymphadenopathy. Mild swelling over joints present with erythema No RASH over body S/E R/S: B/L ronchi with crepts P/A: hepatosplenomegaly grade 2 My impression: Anaemia with hepatosplenomegaly with pneumonia with arthralgia with supraded skin infections with failure to thrive. D/d -Entric fever -Pulmonary Koch — which might have lead to ascitis. -Hepatosplenomegaly with anaemia vs haemolytic with some sec. infection. - Any malignancy?? - collagen vascular Ds Vs vasculitis Investigation: Hb -6 gm TLC 28400, N 80 % Platelets- 469000 PS - from Bhiwani....no abnormal/ premature cell seen, no e/o haemolysis. MP - negative. CRP - strongly positive ESR - 130 Mx-Negative ASO titre -normal Widal- absolutely normal Urine- 40-50 pus cell LFT/KFT - normal Usg- few sub centric mesentric lymphnode seen. After investigation one more thing added - UTI Urine C/S report awaited Till now no final diagnosis has been made...only D/D Any expert opinion ??
