Why

24 yr old ,male, c/0. Recurrent diarrhoea, loss of weight, loss of appetite, No and pain only diarrhoea All gut preparations try but not relieved All test normal except thyroid Help me to find out cause , plan of treatment Patient says thyroid treatment relieve diarrhoea but it occur again and again

He has hypothyroidism It need to be treated Thyroxin 25 mcg per day Hypothyroidism is not the cause of diarrhoea Differential diagnosis Crohn's disease Ulcerative colitis Giardiasis Irritable bowel syndrome Opportunistic infection He needs colonoscopy Please check for immunocompromised status

Hypothyroidism Tsh is very high Start tab thyronorm 100ug 1od keep monitoring and adjust doses Sos iv fluid if pt is dehydrated For diarrhoea try imodium or racecordil Or Rifaximin

Hypothyroidism with chronic diarrhea Hypothyroidism causes constipation ,so it is unlikely to be the cause of diarrhea in this case . Description of diarrhea is suggestive of 1) Irritable bowel syndrome 2) Fat intolerance 3) Adult lactose intolerance 4) Malabsorption syndrome 5) TB enteritis Suggest Stool for reducing substance and fat globules. Pancreatic function test Barium meal follow through / capsule enteroscopy . Colonoscopy

1)Primary Hypothyroidism 2)Start with Thyronorm 75mg OD empty stomach in early morning. 3) follow up for thyroid profile is should be each month for first 2 month then at can be as you need. 4) colonoscopy may be useful to find etiology for diarrhea.
