50year old male with chronic progressive swelling of right lower limb for 20 years..had history of reccurent erythema/pain fever every 2 months..wazza underlying condition and what can be done to prevent reccurent cellulitis?

Elephantiasis .or lymphatic filariasis. Recurrent erythema, pain , fever and cellulitis is known in this condition. As due to constant pressure on skin and subcutaneous tissue , pressure necrosis and subsequent superadded infections are common particularly erysipelas. A course of antibiotics particularly erysipelas and other measures like maintaining good hygiene, elevation of limb , elastic compression stockings can prevent such episodes

It seems more likely a case of elephantiasis.D/d1-elephantiasis 2-lymphoedema 3'-cellutis.advise-1-cbc 2'-color Doppler to evaluate veins and arteries of lower limb.Rx'-Tab Hetrazan 100mg 1×3 2-Tab levocetrizine 10mg 1od3-Tab b complex 1od ×2to3 month.antibiotic and antiinflammatory drugs may be added after awaited reports.if required.
