14 yo girl presents with a painful and intensely pruritic rash in her left axilla that has a macular erythematous base with pearlescent topical papules sprinkled throughout and mild constitutional symptoms. There is mild lymphadenopathy and the contralateral axilla is totally normal. An initial Ddx suspected Shingles, for which Valtrex was prescribed at the appropriate dose. After 5 days, the patient's Mother called to advise that the lesion became a lot worse. A follow-up photos sent by the patient to the office showed a resolution of the vesicular aspect of the rash, but with new mild desquamation as well as increasing confluence of the erythematous base. What do you do?
