64 y/o Male w/ HTN, DM2, OSA admitted with cough, malaise and subjective fevers x 10 days. CXR on admission is posted. The second picture is hospital day 6. He had lymphopenia, normal procalcitonin and mild elevations of ferritin and d-dimer. He is COVID positive. O2 says in low 90’s on 2 L, worsened to 4-5 L NC and then improved to low 90’s on room air at rest by the time of discharge on hospital day 7. The interesting thing about this case is the discordance between his cxr findings and his symptoms/O2 requirements. I would expect worsening symptoms and O2 requirements given the cxr. He received 5 days of azithromycin and hydroxychloroquine.

Slight rotation Tracheal deviation towards right side. Bil Reticulonoduler infiltrations seen. Lobulated aoft tissue opacity seen in rt supraclvicular region. Plz provide other follow up Cxrays and CT thorax images if available.
* Lymphadenitis Obliterans ** PNEUMONITIS WITH CONSOLIDATION Needs further investigation and evaluation to conclude and treatment plan.

SUGGESTIVE OF COVID. POSITIVE NICELY MANAGED...SIR
Patient need to hydrated properly and need large doses of vitamin C for that give 6 glasses of fresh coconut juice and 6 glasses of fresh orange juice. Give 1 glass of coconut juice and 1 glass of orange juice every three hour starting from 6 am till 9 pm first day. Don't give anything else for the first day the temperature will come down to 100 degree. Second day give 3 glassess of each and 300 gms of tomato and cucumber. Now the pateint will feel better. Third day 2 glaasses of each till 12 noon then 300 gms of tomato and cucumber till 6 pm and in the night normal home cooked food. Patient will be symptomless. Please call for more info: 9589637355
It is really good that patient improved in spite of comorbidities Hope his family members must be safe. Corticosteroids are indicated if patient has underlying COPD or Asthma.
