1 month child, stopped feeding
Chief Complaint A 1 month old male child was brought by the parents with the complaint of no feeding and has watery stools with blood. He was only on breast-feeding for 15 days and then stopped feeding. Vitals Vitals are wnl Examination Physical examination shows abdominal distention Investigations Hb: 7.4 g/dl. WBC: Normal C-reactive protein: 16.9 mg/dl. AST: Normal ALT: 144 u/l Albumin: 2 g/dl. C-reactive protein: 3.8 mg/dl X-ray shows descending colon wall thickening & small bowel gas. Treatment What next should be done?
good case . early feed intolerance with positive crp n low hb with abdominal distension n occult blood in stool lays the foundation to suspect nec . whats the platelet count . another close d/d late onset hemorrhagic disease of newborn . need to carry out other investigations too . like pt aptt blood c/s stool for occult blood usg abdomen serum lactate serum electrolytes . admit the child keep npo place nasogastric tube in situ watch for bilous aspirate inj vit k inj piperacillin inj netilmicin inj metrogyl whole fresh blood transfusion iv fluids .
