Why don't we start from the basic diagnosis of ear pathology with mastoid abscess? This is most common following ASOM and should be considered first. A thorough ear otoscopy with drainage of pus is required to begin with.

Possibly a Post Auricular Cyst (infected). Requires Surgical intervention. ( Consistency / mobility also needs to be checked ) DD - Post Auricular Dermoid

Lymphnode enlargement

DD sebaceous cyst

Look like post auricular lymph node enlargement To r /o Koch's Exision biopsy under LA Lymph node biopsy for HPE, AFB,C /S Wound clean with NS Betadine solution T_bact ointment for dressing Tab augmentin 625mg BD Tab pan 40mg OD Tab ibugesic plus tds Adv CBC MT ESR LFT creat X-ray chest Further management as per report

TO EXCLUDE BACTERIAL INFECTION OF LEFT EAR WITH REGIONAL LYMPHADENOPATHY CAN BE TREATED WITH ORAL ANTIHISTAMINES AFTER DINNER, ORAL ANTIBIOTICS AND ANTI INFLAMMATORY/ ANALGESIC COMBINATION PER ORAL, ALL FOR TEN DAYS.

The punctum of sebaceous cyst is visible. Secondary bacterial infection. Antibiotic cover along with excision of cyst after infection subsides
