Here Differential diagnosis are very important in this case. Final diagnosis can be elicited by one by one exclusion method... Contact DERMATITIS ECZEMA Psoriasis Erythema annulare centrifugum Sebhorrhic DERMATITIS

Dr. Dev Singh sir, It is classical picture of TINEA INCOGNITO. Here visual shows extensive inflammation of right side face surrounding by reticular dermis, but still active borders are seen. It could be due to temporary immune suppression by steroid affect. Here the main culprit of fungus is..Microsporum gypseum!!! Diagnosis can be confirm by KOH test. Here fungal hyphae will be clearly visible 3 days after stopping the all the topical applications. Treatment: Abrupt stop of steroids also problem here( rebound phenomenon). So gradually tapering the steroid application by duration and number of times. Gradually replacing by Butanefine or Amorolfine topical application. Cap itraconazole 200 mg after lunch for 3 weeks. First week along with itraconazole, oral retinoids like Isotretinoin 10mg daily morning after breakfast will give good respond. Oral antihistamines to alleviate the itching sensation.
