hardening under big toe. pain while walking. No history of injuries. he use to remove the hardening with knife as it relieves the pain. but again the hard skin regrows and makes painful. plz What is dx and rx?

A male patient 45years old witg p/h/o Itching all over body since last one year. was treated with Anti scabies + Anti fungal by different dermatologists . Present c/o Severe itching, dryness with rough and hard skin on palmer aspects of both foot and hands. my provisional diagnosis is Lichen planus and patient treated with ( Clop-s ) clobetasol+salicylic acid cream/lotion with Anti histaminic... but still no progress. please give opinion about investigation, diagnosis and treatment
