Cellulitis is to be treated with broad spectrum antibiotics. Consider fasciotomy. Get colour Doppler examination of the limb vessels. See for PVD Buerger's disease. See for DVT. Plan accordingly

In limb cellulitis, timely fasciotomy is an important decision to save limb from pre-gangrene / gangrene.

- It's ACUTE CELLULITIS. Of Lt. Lower limb.- pitching odema present at foot. - immediate need COLOUR DOPPLER [ artery & venous both in this case ]of Lt lower limb[ bilateral if CELLULITIS is on both foot.]- RFT- eGfr-Lipid profile- Cbc- HbA1c - Antibiotic preferably cephalosporins group-- NSAID---Antihistamine for 5 day. - further treatment as per investigation suggest

Cellulitis lower limb. Do Colour Doppler examination of the limb and start piperaciline tazobactum combination with chymoral forte and limb elevation and local hirudoid cream.See if swelling reduced or progress.If it progress then do fasciotomy and also stop smoking and do blood sugar examination for diabetes

Since pt is smoker and developing acute symptoms most likely dvt go for colourDoppler and relevent routine investigations and management accordingly till than treat acute symptoms.with antibiotics and analgesics.

D/D 1 cellulitis 2 TAO 3 DVT DOPPLER study as next step

Has pt had any injury,as dursum of foot looks dis coloured. By the picture it looks a case of cellulitis, I can only give limited advise with limited history. X-ray may help. Try ampiclox 500 four times a day .
