Please consider apart from TB, following in view of joint pains SLE. , Lymphoma,. Sarcoidosis. Rheumatoid Arthritis Eosinophilic syndromes Other vasculitis including wegener's Investigation should include vasculitis screen including ANA RA factor,ANCA and abd ultrasound If tests for PTB is negative
Further to the discussion I would think screening for fungal infection specifically Histoplasmosis and coccidioidomycosis may be considered

Apart from pleural effusion, there is no other sign of tb in this Xray. Advice cbc, pleural tap, sputum microscopy and gene xpert test to rule out tb.
Bilateral Pleural effusion Advice Pleural tapping, histopathological examination of Pleural fluid, Sputum for AFB, Start AKT according to reports
