43 year old male patient with H/o plaque form psoriasis since 1999, was admitted around 40 days before for treating exacerbation of plaque psoriasis. he was treated with phototherapy, hyposensibilization therapy and antibiotics. And was discharged with significant improvement after a month. then patient gives history of getting sick with flue like condition few day after discharge (i.e. a wk before today) and has taken Tab. колдакт(pcm+pheniramine+phenylephrine). since last 3 days he has generalised eruption at the same sites where he had previous rashes, but of different clinical form. patient also complaints of swelling in ankle joints (patient has never been diagnosed of psoriatic arthritis). patient also mentioned that these generalised lesions are because he removed skin easily like a sheet from the surface of these lessions and there was pus below crusts before days(his words). patient is subfebrile, with BP being normal. fresh BLOOD tests are not yet available, will post tomorrow as soon as they get ready, 10 days old blood tests were WNL. He is admitted in our clinic under provisional diagnosis. the case is being posted for reaching the right diagnosis and to discuss probable cause for this erruption and to discuss ways of managing this patient...i will post the updates hopefully every other day regarding response to treatment. thank you.
