50+ male patient, asthmatic, returning from a COVID-19-affected European country a few days earlier. Started to feel unwell before departure with fever and shortness of breath. No wheeze, hypoxia, tachypnea or tachycardia on assessment, only pyrexial with persistent dry cough. Arterial blood gas readings are all within range. Would you admit this patient? Would you swab this patient for the virus? Does the fact that he is asthmatic make a difference to your decision making? Would you prescribe antibiotics?
Yes i will advise to admit this pt who is asthmatic First reason is xray chest shows rt paratracheal haziness is pneumonitis Trachea is pulled to rt Peripheral haziness in lt lower zone C/o breathlessness 2 visit to endamic area Yes swab should be taken for Rt pcr for covid19 He is likely positive for covid19
Mediastinal widening , right paratracheal and paracardiac opacity . Right midzone haziness . Cardiomegaly Since this patient has recently has returned from Covid affected European country, he should be quarantined . Since he is symptomatic ,he should be tested for Covid. Should be put on azithral and non steroidal( LABA) nebulisers .His SAO2 should be checked, and if it is below, he should be admitted . If SAO2 is normal , If he develops breathlessness ,he should be admitted .

Every possibility of COVID infection is there. Admit the patient and follow protocols strictly. After quarantine and sufficient period of complete rest to gain adequate immunity required.
