pericardial effusion or cardiomegaly??

57/f c/o b/l pedal swelling f/b facial puffiness, dysponea on exertion,cough with expectoration,orthopoenea since 1 month.no h/o tb,DM, HTN,no h/o any additions.o/e pallor +, edema +,jvp raised,murmer in mitral area present but because of tachycardia it can not be appreciated. On routine examination Hb is 2.9. My question is 1.are these features d/t severe anemia or these are d/t any cardiac pathology. What should be line of management.

ECG AF and low voltage Cardiomegaly hence rule out pericardial effusion,as c/f of ccf very well due to p effusion thou’ aggravated by anaemia .Paracentesis if pericardial effusion followed by treatment of ccf and infusions of packed cells blood.Murmur at heart could be due to anaemia itself,but valvular defects may be possible as cause of CCF and AF.

Low voltage complexes favouring pericardial effusion But anaemia is not the cause of cardiac changes At the same time primary pathology most probably systemic illness leads to both anaemia as well as cardiac effusion

Severe anemia with pericardial effusion 1st ecg shows dysarrythmia 2nd ecg shows tachycardia Start Blood transfusion 300mp with lasix slowly, then after 5 days gap 2nd Continue diuretics 2D echo advice before treatment
