62 y F obese and over wt kco OA both knees comes with h/O nocturnal chet pain with suffocation and prespiration for 6 months she gave h/o taking oha for diabetes since 21/2 yrs now she also has dysphagia to solid food for couple of months x-raychest b/b pt is5month old which suggest some thing her bsl 5month old is absolutely normal todays bslR is 94mg she 75kg and height is less than 150cm thus bmi comes to 36% she is v v poor can't afford detail investigations my questions are 1 since her ecg is absolutely normal reason for nocturnal symptoms. 2 interpret x-ray chest any relation to dysphagia.

Sir you are right ECG is normal But xray showing abnormalities So give advised to Berium meal test Or Endoscopy as per pts condition i think GERD possibility not sure

Yes sir ecg is normal And X-ray also showing abnormality but not cardiac cause as per histry So sir give her advise for Berium meal test for confirmation

As per x Ray it may be diaphragmatic hernia so berium meal for confirmation

Ecg show Low voltage X-ray show Lf Hilar mass Advise CT chest that ruled out both About mass & Dysphagia (Oesophagus pathology)

Xray showing prominent aortic knob ( Calcified) As patient is obese nocturnal chest pain , suffocation rule out obstructive Sleep apnea

R/O mid Lt paratrachial growth which compreses oesophagus

A circular opaque homogeneous opacity in the L mid zone appears to have pushed Trachea to R. is it A cannon ball compressing oesophagus . Is it an FB ?

Thanx sir this s not fb but a suspicious shadow and needs to workout to r/o malignancy still pt has not come in followup .
