Moderate to severe COVID-19 Pneumonia- Treatment obstacles

70 years old male with history of fever and cough since last 1 week . Reluctant to get Nasopharyngeal swab for RT- PCR due to stigma,- attached to the disease. Patient 's attendants taking telephonic consultation asked them to get Oxygen saturation noted at home as reluctant to shift to hospital. It was 88- 90 % since yesterday. Asked them to at least get HRCT -chest - if not RT - PCR- HRCT chest done today showed atypical pneumonia with GGO's and more than 75% bilateral pulmonary parenchyma involved and CORADS 5 lesions . After the CT his PO2 is between 79 - 80 .% . Now they have finally decided the patient to shift to GMC - hospital The Aim of this post is that 1 What the patients of moderate to severe COVID-19 do when hospital facilities are not available in your city and all govt run institutions are fully occupied and no private hospital is available 2.Are in india - we really saving the severe COVID-19 patients and are we able to manage the cytokine storm 3.Considering the social stigma attached with the disease- many of the patients attendants are allowing their old persons at home to die 4.We are fortunate that - mortality in Indian set up is less - may be our immune system is better . It is this factor- which is saving our patients 5.Our health care system has been fully exposed in dealing the severe - COVID-19 patients- who require either non - invasive or invasive ventilation

People arround us are also a little bit Inhumanic and their behaviour are also strange towards the pt even after getting cured and the health care workers what we are seeing in news paper. That is the reason people are scared of getting tested for Covid19 if positive they may be abandant by the society.

It's a tough question to answer More talks with patient and relatives, more feeling of being cared for to they patient can help in decreasing the stigma It is likely that majority of population is going to suffer from covid 19 It's only a matter of time

Our country wth huge population so num of pts ultimately high Anotger point is it is being a social stigma To go for Nasopharyngeal swab nd admitted for covid in all class of peple Nd offcourse healthcare facilities are inappropriate So the things may protect us are our immunity nd self care
