Large endometriosis nodule !!

A 41 y/o/f have a large endometriosis nodule which had grown over time into an episiotomy scar. She had delivered twice 18 and 13 years before, with a right medio-lateral episiotomy performed each time. A year after the second delivery, the patient felt a small swelling at the episiotomy site and ten years later, when she completely abandoned contraceptive pill intake, she observed rapid growth of the swelling, which became larger and more painful during menses. Clinical gynecological examination revealed a solid tender mass 5 to 6cm in diameter, on the episiotomy site. How to deal with this case?

Surgical removal is a better option. Needs further investigation and evaluation to conclude. Multivitamin and antioxidants orally. Wash area with diluted betadine lotion for 3to 4times can be useful. Reassurance and councelling required.

Endometriosis in episiotomy . Do Excision of lesion during menstruation, Because the extent of endometriosis is clearly made out during periods. Send the tissue for HPE.
It is endometriosis nodule Study the extent of lesion If it is confined to episiotomy scar it can be removed

Complete excision under anesthesia would be curative

Excision of the nodule should be curative.
