A 29 y/o female presents with hematuria and right-sided flank pain for 4 days. Radiography shows a 10 cm right mass in the right kidney diagnosed to be renal cell carcinoma (RCC) along with hilar lymphadenopathy. She also had multiple lesions in the liver. FNAC shows metastases. Advise further steps.

RADICAL Nephrectomy. Secondary or metastasis may be drained out and after further investigation and evaluation chemotherapy or radiotherapy is the option under oncologist evaluation and opinion.

First step to be taken is of Radical Nephrectomy. As secondary drained to multi organs, chemotherapy and Radiotherapy should be initiated under strict supervision of Oncologist.

Obviously Radical nephrectomy , histopathology and chemotherapy/ radiotherapy as per advise of Oncologist

Adv PET scan. Chemotherapy. Sos surgical intervention.

Refer the patient to a medical oncologist. Sometimes radical nephrectomy can improve survival even in a patient with metastatic disease. However, it depends on the morbidity of surgery, the renal mass size, and the amount of disease outside the kidney. Because the patient is so young, hereditary renal cell carcinoma syndrome (VHL syndrome, Tuberous sclerosis) needs to be considered in this patient. this patient would be treated with Lenvatinib/axitinib+ Pembrolizumab (if affording) or Lenvatinib+ everolimus (if non-affording)

Renal cell carcinoma with metastasis Adv Chemotherapy as per oncologist advice Usually Sunitinib is drug of choice

Redical Nephrectomy followed by Chemotherapy + Radiotherapy under supervision of an Oncologist
